There are links to the left to take you to the forms that you need if you have scheduled a session. The forms are set up to either submit online or to print and bring with you.
~If this will be your 1st session, you will need both the New Client Form and the Consent Form. Returning Clients only need the Returning Client Form.
New!!! - - - I now use a sliding scale plan for payments! Pay between $10 - $40 per session depending on what you can afford!
